We’re glad you’re here. After scheduling your appointment, please complete the new patient form below.

We know every child is different. This form takes 10–15 minutes and helps us prepare for your child’s visit.

What to Bring to Your Appointment

  • Insurance card  (if applicable)
  • Parent or legal guardian
  • Completed new patient form (if not submitted in advance)

Insurance & Payment Information

We aim to make paying for care simple. We accept in-network and out-of-network insurance, as well as self-pay.

Our in-network providers include: Met Life, Delta Dental Premier, Cigna, United Concordia DoD, and Superior/Medical Mutual. 

Our Office also accepts CareCredit payments. CareCredit is a financial assistance program available to those with or without insurance.

  • Email the completed form to our office prior to your visit, if possible

  • Or print and bring the completed form with you to your appointment

If you’re unable to complete the form in advance, please plan to arrive 20 minutes early.
